Case Manager, Family Medicine

The Case Manager is responsible for case management and community outreach with criminal justice-involved or formerly criminal justice-involved individuals with co-occurring substance use and mental health problems in the Opioid Intervention Court Expansion Project, using the MISSION-CJ (Maintaining Independence and Sobriety through Systems Integration, Outreach, and Networking – Criminal Justice Edition) evidence-based model.About UBThe University at Buffalo is SUNY’s most comprehensive public research university, and an outstanding place to work.
